2 = Satisfactory        Acceptably Fair for Grandparent(s) and Child(ren)
3 = Unsatisfactory – Very Unfair to  Grandparent(s) and Grandchild(ren)
4 = Unsure –             Undecided – Still in progress, waiting for outcome.

 Note:  If you do not know the names of “Other Professionals” , i.e., “Police” or “Security Guard” etc., just list their profession, location and rating of our encounter
